V8 Clean Operator UI

Changes:
- Tournament page separated into sub tabs: Setup, Teams, Groups, Recovery.
- Fixtures moved to a separate main tab.
- Fixture cards show real team names with Start Match buttons.
- Live match cards show Open Live, Start 2nd Innings, Complete.
- Completed matches are locked visually.
- Advanced group/recovery options are no longer mixed with match cards.
- Recovery manager remains optional and can be used anytime after opening matches produce losers.
- Replay module is kept.

Recommended flow:
1. Tournament > Setup: Create/select tournament.
2. Tournament > Groups: Assign teams and generate fixtures.
3. Fixtures: Start matches from cards.
4. End first innings in live scoring, then use Start 2nd Innings from live or fixture card.
5. Complete match to lock card and auto-advance bracket.
6. Tournament > Recovery: Add selected losers and generate recovery fixtures when needed.
